Hourly TrueCare was established in 1971 with a mission of providing

ability to pay. The Medical Assistant (MA) is responsible for
performing routine clinical and administrative tasks. Clinical
duties may include taking and recording vital signs and medical
histories, preparing patients for examination, drawing blood, and
administering medications. Administrative duties may include
answering phone calls, registering new patients, scheduling
appointments, and collecting and posting fees.POSITION SUMMARY The
Child Development Specialist is an early childhood development
expert who will join the pediatric primary care team at TrueCare to
provide interventions, referrals, and follow-up for families with
patients ages 0-5 years. The Child Development Specialist builds
strong relationships with families and providers to increase the
efficiency of the medical system and support team-based
comprehensive care.ESSENTIAL FUNCTIONSPreventative and Responsive
Care
- Conducts and provides team-based Ages and Stages Questionnaires
(ASQ) during the age specific well-child visits before or during
the provider. Visit with the patient via manual or ASQ online
digital platform with a focus on monitoring development,
social-emotional functioning, and relational health, and providing
anticipatory guidance.
- Conducts consultations and works with parent/guardians to
complete the Ages and Stages age-appropriate questionnaires (ASQ)
prior or during the Well Child visit with provider.
- Will perform direct observations on pediatric patients 0-5
years to assess development and will document assessment in E.H.R
and report back to the care team to identify appropriate
intervention, resources, and/or follow-up.
- Participates in care team huddles and reflective supervision
meetings.
- May facilitate parent/caregiver groups. Referrals and Community
Collaborations
- Creates and maintains community resource directory/database.
Provides referrals and tracks follow-up, as appropriate, to help
families make successful connections to key resources within the
community.
- Reaches out to community providers within first 6 months in
position to share about Ages and Stages, their role, and receive
information to support referrals, warm handoffs, and ensure
appropriate referral criteria is met.
- Participates (when appropriate) in community-wide early
childhood and/or mental health meetings. Team-Based Care
- Is an active member of the ASQ Implementation Team and attends
regular team meetings.
- Works with the front office staff to ensure scheduling of the
Child Development Specialist's time in coordination with provider
well-child visits to administer the Ages and Stages (ASQ)
questionnaires.
- Engages in Continuous Quality Improvement initiatives to ensure
the ASQ model is delivered with fidelity.
- Collaborates with peer Child Development Specialist(s) and
patient care teams to implement the core components of ASQ,
including universal screenings, making positive parenting and early
learning guidance information available, and adjusting workflows
for optimal efficiency.
- Maintains open communication with all members of the primary
care team.
- Works closely with pediatric primary providers around care
coordination, goal setting, coaching, and education about key
aspects of a child's development.
- Provides consultation and/or facilitates training to medical
professionals and all practice staff re: early childhood
development, early relational health/infant early childhood mental
health, and trauma-informed practice. Caseload Tracking, Record
Keeping, and Reporting
- Maintains extensive databases required to meet ASQ fidelity
metrics including both internal and external referrals.
- Documents all patient clinical activity and care coordination
in the E.H.R.
- Tracks caseload to ensure capacity to deliver services within
the risk stratified service delivery model. This includes ensuring
exit criteria is met and that families and children are elevated to
higher levels of care or referred to community providers if needed.
QUALIFICATIONS REQUIRED
- Bachelor's degree in psychology, social work, counseling, early
childhood education, or relevant field.
- Experience and knowledge about early childhood growth and
development, parent-child relational health, infant and early
childhood mental health, and family systems.
- Bilingual in Spanish, both written and verbal.
- Availability to travel and/or work occasionally outside of
normal shifts (e.g., 8am to 5pm).QUALIFICATIONS DESIRED
- Master's Degree in psychology, social work, counseling, early
childhood education.
- Clinically licensed mental health professional.The

(within the pay range), will be determined based on factors such as

is the full base salary range for the role. Hiring at the maximum
of the range would not be typical in order to allow for future and
