Job Description TrueCare Job Title: Patient Services Advocate Location: Oceanside, California. We'd love to hear from you Position Summary: The Patient Services Advocate is responsible for identifying designated groups of patients who require education and direction for obtaining primary care services. This includes initiating contact and developing appropriate relationships with an assigned group of patients to increase appropriate access to primary care services; increase compliance with follow-up for a specified condition, diagnosis, or procedure; and/or decrease inappropriate use of urgent care and emergency services. ESSENTIAL FUNCTIONS: Extract reports from various sources, including TrueCare reporting software (Practice Analytics), Health Plans, and the Pulse, as needed, to identify patients for health care. Follow-up with patients in an assigned group or population to schedule appointments (i.e., well care visits, ER follow-ups, dental exams) according to scheduling guidelines and the empanelment process, consistent with the Patient Centered Medical Home and Team Based Care models. Provide referrals and resources as appropriate and assist patients in navigating within TrueCare. Connect with patients and inquire about general health as directed. Build/maintain monthly productivity reports. This includes managing the patient recall process and outreach to patients to schedule appropriate follow-up appointments. Implement and manage the appointment reminder process, including but not limited to reviewing confirmation reports and cancelling appointments in the system as necessary. Maintain the cancellation voicemail line by consistently clearing voicemails and cancelling appointments in the system as necessary. Provide access and coordination of care for patients in need of hospital or ER follow-ups to engage them in care. Manage the population based outreach line and schedule appointments according to clinical and scheduling guidelines. Prepare and mail patient letters as directed. Qualifications Required : High School Diploma or equivalent. At least one (1) year of call center, receptionist, or Medical Assistant experience in a health care setting, or three (3) years of customer service experience. Computer literacy working with PC-based software applications and proficiency with the Microsoft Office suite, including Outlook, Word, PowerPoint and intermediate skill level in Excel. Bilingual in Spanish Qualifications Desired: Associate---s Degree in health sciences or related field. Working knowledge of medical terminology and medical center workflow.
