Health Outreach Worker is responsible for providing outreach and
education services focused on health awareness to improve health
literacy, healthcare access for under-served communities and to
increase knowledge among the community. The Community Health
Outreach Worker also works collaboratively with TrueCare
departments as well as community-based agencies to make critical
linkages to TrueCare health information and services for potential
clients and patients.ESSENTIAL FUNCTIONS1. Coordinates and/or
attends community events such as health fairs, food distributions,
community flu shot events and community forums, focused on
providing resources, health awareness, enrolling new patients and
increasing visits.2. Distributes informational material to targeted
communities that describe what, where, and when program services
are available to them.3. Coordinates with support staff across
clinics to offer on-site appointment availability during outreach
activities.4. Builds and maintains rapport with community based
organizations and community members.5. Support grant programs to
develop and implement activities to successfully achieve project
based outcomes while also implementing data collection activities
and report submissions.6. Develop health education content for
community presentations, including lesson plans, Power Point
presentations, supplemental materials, engagement activities and
additional resources and seek collaboration with knowledge experts
both internally and externally.7. Works with identified community
partners to deliver educational presentations and information
related to TrueCare services as well as identified health education
topics e.g. substance use, nutrition, dental health, etc.8. Perform
blood pressure screenings and height and weight assessments during
outreach activities as appropriate.9. Conducts community
presentations and other needed outreach activities to reach Spanish
speaking low income medically indigent people in North San Diego
and Riverside County.10. Conducts outreach, engagement and health
education activities through participating in community events by
frequenting and visiting community based agencies and
organizations.11. Maintains a database of current referral
resources in order to link community outreach contacts with
appropriate services.12. Maintains accurate and thorough
documentation of outreach activities, and assists in tracking data
related to community events and presentations to patients and
community members.13. Maintains inventory of health
education/outreach materials necessary to conduct outreach efforts
and to supply internal providers and other staff as needed.14. Data
collection, whenever possible to confirm participation in
activities and to follow up with potential patients.15. Attends and
participates in all appropriate staff meetings, as
directed.Qualifications:Associate's Degree in healthcare or related
field or equivalent work experience. At least one (1) year
experience in health care or related field performing outreach,
education, case work, and/or customer service to diverse
populations. Knowledge of local community-based
agencies.Availability to work regularly outside of normal shifts
(i.e., 8am to 5pm).Public speaking, computer literacy, including
internet navigation and research skills, and proficiency with the
Microsoft Office suite, including Outlook, Word, Excel and
PowerPoint. Clean driving record, proof of insurance and CA drivers
license - no tickets/accidents- will be driving to venues, need
reliable transportation. QUALIFICATIONS DESIREDBachelor's Degree in
healthcare or related field.Bilingual in English and Spanish.Local
candidates will be considered LICENSESWill be required to travel to
all NCHS' sites and community locations therefore California
Driver's License, proof of insurance and clean driving record
