We'd love to hear from you!Position Summary:The Resource Nurse is responsible for assisting in policies, protocol review, updates, and coverage for clinical site areas to ensure the highest standards of patient care. The Resource Nurse will provide support for TRUECARE' Community Engagement department through community screenings, vaccinations, and health education. In addition, the Resource Nurse will assist with Quality Assurance chart reviews in adherence to NCQA guidelines and ensuring compliance with regulations such as Title XXII.ESSENTIAL FUNCTIONS:1. Researches and analyzes best methods and practices relative to achievement of specified goals and operating objectives.2. Assist in developing process workflows as it relates to assigned projects and/or to support internal process improvement initiatives.3. Participates in and leads various task forces to improve work flows and enhance the patient experience.4. Assist in identifying training needs of medical assistant staff and nurses on an as needed basis and act as preceptor for Medical assistants and/or nurses.5. Assists in monitoring clinical and department goals and objectives.6. Assist with Quality Assurance chart reviews in adherence to NCQA guidelines and ensuring compliance with regulations such as Title XXII.7. Participate in in-service training as required.8. Responsible for patient centered coordination of health care services.9. Implement case finding techniques to identify high risk individuals.10. Assist families and individuals to maximize all resources for their health and well-being.11. Recognize conditions that signify gaps or failure in the coordination and preventative effort of case management and respond appropriately.12. Coordinate health education services to individuals and families based on need.13. Responsible for ensuring appropriate referrals and punctual completion as needed.14. Participate in the collection of quality monitoring data.15. Maintain accurate records regarding referrals and perform quality management follow-up.16. Produce monthly audit reports as required.17. Prepare patients to be seen, taking history, blood pressure, weight, temperature, etc. as necessary.18. Give direction to medical assistants regarding procedures and protocols as required.19. Responsible for administering lab procedures, EKG's and other tests as well as keeping records of tests and filing results.20. Dispense selected physician ordered medication according to Board of Pharmacy regulations.21. Administer ordered medications according to current health center protocols.22. Display knowledge of normal signs of human development and ability to assess and provide age appropriate care.23. Provide support for TRUECARE' Community Engagement department through community screenings, vaccinations, and health education.24. Provide Support to employee health when needed.Qualifications Required : High School diploma or equivalent.Graduate of an accredited California School of Professional Nursing.Two (2) to Four (4)) years' clinical nursing experience in an outpatient care setting or ambulatory care.Bilingual in SpanishComputer proficiency, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook and Word.
